The University of Ilorin Teaching Hospital (UITH) is now accepting applications for its School of Health Information Management (SHIM) ND and HND programs for the 2022/2023 academic session. The UITH SHIM admission form is now available online. See below for the requirements and how to apply for the form.
Applications are invited from suitably qualified candidates for admission to the School of Health Information Management’s National Diploma (ND) and Higher National Diploma (HND) programmes (full-time) for the 2022/2023 session.
Course duration: two (2) calendar years beginning in October 2022. (Full-Time Programme).
The course is non-residential in nature. However, there is limited space available on a first-come, first-served basis.
Admission Requirements for UITH SHIM ND Programs
Applicants must have taken the 2022 Joint Admissions and Matriculation Board Unified Tertiary Matriculation Examination (UTME). The School will determine the minimum score.
In addition to the 2022 UTME, which requires three subjects from English, Mathematics, Biology, Physics/ Chemistry, and one (1) other subject.
Candidates must also have a West African School Certificate/General Certificate of Education/SSCE/NABTEB/NECO with 5 credit passes in five (5) papers obtained in no more than two (2) sittings. English Language, Mathematics, Biology/Health Sciences, Chemistry/Physics, and any other 1 subject from Economics, Government, Geography, Religious Knowledge, and any Nigerian language must be included in the papers.
Admission Requirements for the HND Program at UITH SHIM
Applicant must meet the National Diploma Admission requirements and have a National Diploma Certificate or a Health Records Technician Certificate.
Candidates with ND certificates in Health Information Management must show proof of one year of industrial training and a professional license certificate issued by the Health Records Officers Registration Board of Nigeria (HRORBN).
Candidates must also have a West African School Certificate/General Certificate of Education/SSCE/NABTEB/NECO with 5 credit passes in five (5) papers obtained in no more than two (2) sittings.
These papers must include English, Mathematics, Biology/Health Sciences, Chemistry/Physics, and one other subject from Economics, Government, Geography, Religious Knowledge, and any Nigerian language.
How to Apply for Admission to the UITH School of Health Information Management
The application forms are to be downloaded from the University of Ilorin Teaching Hospital website (www.uithilorin.org.ng) and a non-refundable fee of N7,000.00 is to be paid into the University of Ilorin Teaching Hospital Account with the CBN via the CBN e-payment gateway REMITA.

After making payment through the Remita Platform, you can complete your registration by filling out the online SHIM FORM with the Remita Retrieval Reference (RRR) Number generated with the payment.
When you click APPLY NOW, you will be redirected to an interface where you will be asked to enter a VALID personal E-mail address.
A confirmation link will be sent to the E-mail you provided during the E-mail registration.
Login into your E-mail account by opening a new Tab in the same browser already opened.
Click on the link sent to your E-mail by noreply@uithilorin.
The SHIM application form will be made available for you to fill and submit.
Select the appropriate programme you wish to apply for in the drop down menu.
Fill every other field on the form as required.
After submission wait for 2 seconds as you will be directed to print or download the filled form.
NOTE: You are to bring the online printout after submission on the day of exam/interview.
